✦ “P” stands for Previous suicide attempt. If someone’s crossed that line before, it’s much easier for them to go there again.
✦ “R” stands for Rational thought loss. Mental illness is real, and mentally ill people can easily lose touch with reality. They may hear voices telling them to kill themselves, and they may listen.
✦ But the big one, hidden in the ten where you would never find it unaided is “O.” Organized plan. People don’t commit suicide lightly, and it’s rarely spontaneous. They practice and rehearse. They walk out on the bridge and look down, or they hold the gun up to themselves. If you’re a therapist looking for clues, the presence of a plan is paramount. In fact, so important was this one predictive indicator that it became the load-bearing insight of Aaron’s pyramid and presentation, and he crafted its language carefully. It’s a great example of what an insight should look like. The insight was: “The key to suicidality lies in the inner world of a person’s concrete plans to die.”
